Risk assessments 

Why: risks assessment exist so that you yourself and the crew that you have with you understand the risks and where they are how they can be controlled or prevented and how to handle the situation.

How: You fill out a risk by first deciding the hazard reference which you choose one of the options from a to m in the very top bar at too the page then you fill out a hazard description describing in detail what the hazard is then you put in the next box how many people are at risk and what there job role is (e.g. 2 actors) then you fill in the next three by looking at the S L R s is the severity which you decide between 1 and 4 and then you fill out the likelihood with 1 and 4 again and in the last box you times the pervious 2 together then the next box is the control measure and that is how yo control the hard you are describing then you refill in the S L R again to see how much better it would be with the control measure you filled in and the action comments is if you decide if anything else needs to be done.

1 2 3 normally you go for last take because it will be best but the other takes are salvageable or useable the best will be circled which is the best way to find best when looking through the shot log to edit
